Any advice on questions I should ask other than these?!
1. Risks of surgery
2. Recovery time
3. Size/location of scar
4. Why is surgery better than "wait and see"
5. Cost and how to get insurance to cover it (that was part of my email questions for him and I know it may be someone else I talk to about this)

After care instruction: not only time of recover, but what you might see and have to care for.
What they expect to do, (things can change once surgery begins... they might find they need to take more/less close it differently, etc.)
Other options, what to expect long term, short term.
Do you have examples to show me of similar cases?

Looks like you have most of it covered. Dr. Levitin is VERY thorough and will most likely answer your questions before you even get a chance to ask them. He's great and easy to talk to as well.
