Ulcerated diaper area hemangioma
Hello everyone. Our 6 month old daughter has a diaper area hemangioma which ulcerates and causes her a great deal of pain. She has begun propranalol treatment, which has flattened and smoothed the area, but the ulcers are still forming. We are also using wound treatment protocol and loads and loads of Aquaphor. We are considering trying laser therapy next. Any feedback?

Hi and welcome!

No feedback here, except to say that it sounds like you are doing what you're supposed to do. I know Dr. Serena says that ulcerations must be kept moist, but not too moist, and it's not easy to get the right balance all the time.

Write to Dr. Serena if you need to: He's listed as the Wound Care Expert. Also, write do Dr. Nelson with questions about laser treatment, if you have any.

Yes, we've had many parents that ended up having laser tx. for stubborn ulcerations especially in the diaper area over the years. It really is a quick procedure and the results have been very good for healing the ulcers. My son had a small ulcer on his ear... small put painful! It was so hard to dress him... getting anywhere near his ear with clothing felt like I was killing him. One zap with laser and it was gone. I can't even see the area where it was... ulcers tend to scar... but the laser sure helped.

My daughter had laser treatment and for her it was a very positive experience. Her hemangioama was large and covered the back of her head so we had the problem that lying on her back, sitting in her baby car seat and even putting clothes over her head caused her lots of pain. The only negative part of the laser treatment is that she now has a scar and a bald patch because of the laser treatment but we were told some scarring and baldness would probably occur anyway due to the scarring that occurs naturally when a wound repeatedly breaks down.
I was told the laser does hurt but my daughter did not cry or fuss when it was given, (maybe thats because she was so used to experiencing pain). The laser changed the colour of her hemangioma (the lasered area turned white like any other scarred skin you might have). It took about 2 weeks (and 2 goes with the laser) to go from being a painful infected wound to being pain free and completely healed. Laser made a big difference to my daughter and I because she was no longer in pain, so if you decide to go ahead with the laser treatment i hope you have as positive an experience as we did.

My daughter has a diaper H as well.
My daughter is now 10months and is still on propranolol. She has a quarter size above her buttocks and some on the sides. The one that worries me most and causes pain is the one in btwn the cheeks. It gets ulcerated right above her anus. That one healed but recently another spot ulcerated. We're using regranex for the ulcerations and it's helping a lot but very slowly. I haven't found much information about laser improving Hemangioma's in the diaper area. Please keep us updated about your child's H.

My daughter had laser treatment for her ulcerated hemangioma on her left butt cheek in 2009. She received 4 or 5 treatments every two weeks. It kept the wound clean and allowed it to heal faster. We used mepilex which is a waterproof bandage that worked well. The only other thing we put on it was vaseline. She has a scar from the ulcer & laser, but I believe we made the best decision for her at the time. We were thankful to not have to put her on medication. She still has her hemangioma and it is SLOWLY fading. She turned 2 yesterday! Laser is a great option for ulcers. We did have to drive to over 4 hours for her treatments.
